Job Title: RN Case Manager Profession: Registered Nurse Specialty: Utilization Review Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum of 2 years required License: Valid State RN License required Certifications: AHA Certifications in hand at the time of submission Must-Have: - Specific charting experience required, Epic experience preferred - Background policy to be noted prior to submission Description: Coordinates utilization management, resource management, discharge planning, post-acute care referrals, and care facilitation. Oversees the management of acute patient populations across the care continuum. Focuses on providing coordinated and integrated care to prevent unnecessary admissions or readmissions. Provides discharge planning coordination and intervention for high-risk patients presenting to the Emergency Department. Strives to promote patient wellness and improved care outcomes. Aims for efficient utilization of health services and minimization of payment denials. Works with a patient population that has complex health needs.
