() Job Details Overview Dignity Health's Pacific Central Coast Health Centers (PHC) is a non-profit Community clinic organization comprised of more than 40 health centers located primarily from Ventura to Templeton spanning the coast. Our experienced physicians and advanced practice providers offer a wide range of primary and specialty care services. PHC is a part of Dignity Health Central Coast's award-winning network of outstanding hospitals imaging centers laboratories and post-acute services. Responsibilities The patient navigator has a unique role in patient care. As a patient advocate, this individual will build working relationships, solve problems, direct patients to resources both within the organization and externally in the community as well as support patients as they learn to self-navigate the health care system. The primary activity of the Patient Navigator is to call patients following their visit or hospitalization to discuss the experience, support the patient through the use of community resources and acquire a primary care provider, as well as a specialist where indicated (e.g. Orthopedist for fractures).
